Welcome to L&I

To bill for services, all providers must apply for an L&I provider account number through ProviderOne.  All healthcare providers must have a National Provider Identifier (NPI) to apply.  If you need to obtain an NPI, see the "NPI requirement" tab below. 

    For the following provider types use the "Exempt Provider Application" tab below: 

    • Interpreters
    • Independent medical examiners
    • On the job training (OJT)
    • Veterans Affairs (VA) facilities
    • Third party pharmacies

    Apply with ProviderOne

    ProviderOne is an online portal where you can submit an application to enroll with multiple state agencies.  

    All providers new to L&I and ProviderOne must apply for an L&I account through ProviderOne

    Forms required when submitting your application:  
    If you practice in Washington State and are one of the following provider types, you are required to be in network to be paid for services beyond the initial visit.  And, if you are new to L&I, you must upload the following forms in ProviderOne when you apply. 
  • Physicians

  • Note: These documents are required on the initial enrollment. Once a provider is approved in network, the malpractice insurance is the only document that must be uploaded, when added to a new group. Billing providers are also required to submit a copy of their IRS Form W-9​.

        Accessing Your ProviderOne Account

        All Healthcare providers in Washington, Oregon, or Idaho, visit OneHealthPort (OHP) to access ProviderOne.

        • If you have a OneHealthPort account, your organization’s OHP administrator will connect ProviderOne to your account.
        • If you don’t have an account, you’ll need to register using your Tax ID.

        Providers outside WA, OR, or ID and all non-healthcare providers 

        1. Go to Health Care Authority's (HCA) Provider Contact Us web form.
        2. Select topic: "ProviderOne Access Request Form".
        3. Complete all required fields.
          • If you do not have an NPI, enter 000000000 in the NPI field.
        4. Submit the request.

        If you are exempt from ProviderOne, follow the instructions below to apply.   

        Here's how to apply:

        1. Complete a Provider Account Application (F248-011-000).
        2. If you are one of the following provider types, you have additional requirements. To fulfill those requirements, visit:

        4. Avoid payment delays! Go to ofm.wa.gov/payee and submit the Vendor/Payee Registration form directly to the Office of Financial Management (OFM) for: 

        OFM forms must be submitted at the same time as your Provider Account Application. 

        Note: Providers are responsible for directly submitting necessary forms to OFM. L&I can’t accept or forward OFM forms on behalf of the provider.

        Next Steps

        Allow 60 - 90 days for application review. You will receive notification once your application has been processed. If you haven't heard from us after 90 days, please email PACMail@Lni.wa.gov.

        ProviderOne Action Required

        Action Needed:
        In order to ensure there are no billing issues with your group or servicing accounts, please complete the following steps.

        First, check to see if the Provider exists in ProviderOne:
        The easiest way to determine this is by clicking into Step 14 from your group domain, click “Add”, enter the individual providers NPI and start date, click “Confirm Provider”.

        • Proceed to Option 1 if you get an error message that states “this NPI doesn’t exist in ProviderOne”
        • Proceed to Option 2 if the system prompts you to add an agency.

        Option 1: To add individual servicing providers who do not exist in ProviderOne, follow these steps:

        1. Click to Apply for an L&I account through ProviderOne.
        2. Choose individual provider enrollment.
        3. Please use individual provider's Social Security Number (SSN) and National Provider Identification (NPI).
        4. For servicing type you must select “servicing only”.
        5. Complete all required steps for this enrollment by using this guide.
        6. Only select the primary taxonomy for the individual. L&I only requires the primary taxonomy.
        7. Step 15 of the providers’ application is where you add the group NPI (type 2) to associate the provider to your group.
        8. Step 19, the final step, indicates all required documents you need to upload before you submit. Make sure all documents are complete and signed.
        9. Click "Submit Enrollment" to send to L&I for review.  

        Option 2: If the Individual Provider exists in ProviderOne, follow these steps:

        1. If the Provider is found, you will be prompted to add an agency.
        2. Follow the system prompts.
        3. Once you have completed the association, stay in Step 14 and filter by Status I%.
        4. Click on the providers name to complete the enrollment by using Step 14 of this guide.
        5. Review each required step and upload the required documents at the final step.
        6. Submit new application modification for L&I review.
        7. Repeat steps 1-6 for each additional provider. 
        8. Go back into the group and submit modification once you have completed your updates.

        All healthcare providers who are HIPAA-covered entities (individuals or organizations) must obtain a National Provider Identifier (NPI) before they can apply in ProviderOne. 

        If you do not obtain an NPI you cannot start an application in ProviderOne. Providers must have an active account to bill for services.
