Appeal L&I's Decision

Instructions for appealing a decision.
	If L&I has turned down your protest, you, your doctor and your employer have the right to appeal to the Board of Industrial Insurance Appeals (BIIA).
Questions workers have: What information should my appeal contain? If you choose to appeal a decision L&I has made about your claim, your written appeal must contain: Your name and address Attorney's name and address, if you have retained one Employer's name and address Claim number Description of your injury/disease The date it occurred Your reason for disagreeing with L&I's decision The relief you are requesting from the BIIA City in which you wish the proceedings to take place Where do I send my appeal? Send your written appeal to: Board of Industrial Insurance Appeals PO Box 42401 Olympia, WA 98504-2401 Submit your appeal online to BIIA (www.biia.wa.gov).
