Employer Claim Notification
To notify employers right away when an employee files a workers' compensation claim, L&I will send notifications to:
- The claim mailing address. The address you asked us to use for all claims-related L&I paperwork.
and, if it's different from the claim mailing address, - The physical mailing address. The address of your company's physical location.
Contact your account manager at 360‑902‑4817 if you receive duplicate copies of claim arrival letters.
