Course accreditation process
Sponsors and instructors
Submit a Self-Insurance Continuing Education Sponsor/Instructor Application for Course Approval form and the supporting course information to Labor & Industries.

Certified claims administrators
To apply for credit for a course taken that hasn't been approved for credits, submit a Self-Insurance Continuing Education Application for Course Approval and Attendance form (F207‑206‑000), supporting course information, and a copy of your certification of completion to Labor & Industries.
See each form for additional instructions. Submit all supporting documentation available regarding the course:
- Copies of PowerPoint slides.
- Handouts.
- Speaker notes.
- A detailed description of the course including the training method.
- A course agenda showing detailed time allotments.
- Copies of course texts.
- Instructor biographies.
- For webinars a copy of the webinar or access to the webinar.
Approval and credit determinations will be based solely on the documentation submitted.
The supporting documentation should be submitted in a reasonable format and order. Your application will be prescreened for completeness, and may be returned to you if it lacks all the necessary information.
Note: If returned, you must resubmit the entire package.See Curriculum Review Committee for additional information.
