See each form for additional instructions. Submit all supporting documentation available regarding the course:

  • Copies of PowerPoint slides.
  • Handouts.
  • Speaker notes.
  • A detailed description of the course including the training method.
  • A course agenda showing detailed time allotments.
  • Copies of course texts.
  • Instructor biographies.
  • For webinars a copy of the webinar or access to the webinar.

Approval and credit determinations will be based solely on the documentation submitted.

The supporting documentation should be submitted in a reasonable format and order. Your application will be prescreened for completeness, and may be returned to you if it lacks all the necessary information.

Note: If returned, you must resubmit the entire package.
