Medical Provider Network Status Lookup

Check the status of a provider that has applied to be in our network.

For providers who submitted applications by December 31, 2012: You can continue to treat injured workers while we review your application. We will confirm this by mail and will also notify you when our review is complete. If we do not approve your application, you will receive 60 days' notice and can continue to treat injured workers during this transition period.

NPI L&I Provider ID  :

• Special note for emergency department and certain hospital-based providers

  • If you treat injured workers exclusively in a hospital emergency department, you do not need to apply to L&I's network.
  • You do not need to apply if you practice solely in an acute care hospital and are a radiologist, pathologist, certified registered nurse anesthetists (CRNA) or anesthesiologist, (other than in pain management practice).
  • However, if you also practice in a non-hospital setting, you must join the network to treat injured workers beyond an initial visit.
