Register your National Provider Identifier with L&I before you bill us

If you don't have an NPI, apply through CMS.

If you have an NPI, to register with:

• L&I: Call Provider Accounts at 360-902-5140.
• CVC:Call CVC Provider Accounts at 360-902-5386.

If you are a new provider, get an L&I provider account number:

• If you'll see injured workers, use the Provider Account Application (F248-011-000).
• If you'll see crime victims, use the Provider Account Application - CVC (F800-053-000).

Note: Each application includes an area to record your NPI information.
