Provider Enrollment

Enrolling a provider

COHEs identify, recruit and enroll providers in their service area. A provider is required to have an L&I provider account number and an orientation to COHE before they complete the COHE supplemental application. Please submit the completed form to your L&I contract manager.

For verification of provider enrollments, L&I contract managers will provide each COHE an enrollment report that lists all participating providers on a quarterly basis beginning October 2013. You can also request an enrollment report from your contract manager.

Dis-enrolling a provider

Those providers requesting removal from COHE should complete the dis-enrollment form. Submit the completed form to your L&I contract manager.

For involuntary APP dis-enrollment, please contact your L&I Contract Manager to discuss the process.

Conducting provider orientation

Orientations can be done in person or through technology as long as it is possible to verify that the provider did watch the presentation. Here are examples of provider orientation slides and script:
