If there's a state government shutdown due to lack of a 2017 – 2019 Washington budget, many services provided by the Department of Labor & Industries would not be available starting July 1. L&I has a full list of service interruptions should there be a full or partial shutdown.

Provider Enrollment

Enrolling a provider

COHEs identify, recruit and enroll providers in their service area. A provider is required to have an L&I provider account number and an orientation to COHE before they complete the COHE supplemental application. Enrollment of a provider can be completed using the form below or through the Occupational Health Management System (OHMS). Please reference the OHMS section of the COHE Toolkit for more information.

For verification of provider enrollments, L&I contract managers will provide each COHE an enrollment report that lists all participating providers on a quarterly basis. You can also request an enrollment report from your contract manager at any time.

Dis-enrolling a provider

Voluntary dis-enrollment of a provider can be completed using the form below or through the Occupational Health Management System (OHMS). Please reference the OHMS section of the COHE Toolkit for more information.

For involuntary APP dis-enrollment, please contact your L&I Contract Manager to discuss the process.

Conducting provider orientation

Orientations can be done in person or through technology as long as it is possible to verify that the provider did watch the presentation. Here are examples of provider orientation slides and script:
