Acupuncture - Pilot Project


Assessment and Monitoring

As part of the requirements for participation in the L&I acupuncture pilot project, acupuncture providers will be required to submit two separate functional questionnaires. For more information, view how to meet these documentation requirements.

Required Documentation- Assessment using 2-item Graded Chronic Pain Scale (2-item GCPS)

Use the 2-item GCPS at baseline, at the middle/fifth visit, and on the final visit to track and document the worker's functional status:

Required Documentation- Assessment using Oswestry Disability Index (ODI)

Use the ODI at baseline, at the middle/fifth visit, and on the final visit to track and document the worker's functional status:

Required Reason for Discharge

On the final visit, the reason for discharge of the patient must be documented, from the following list:

  • Goals met.
  • Lack of progress.
  • Patient/ caregiver request (self-discharge).
  • Medical complication.
  • Frequent no show/ cancellation (noncompliance).
  • Insurance benefits expired.
  • Care transfer (other facility/ level of care).
  • Other (please describe).
