The following policies will be discontinued on December 31, 2018:

Antiepileptic drugs (AEDs) for neuropathic pain
Duloxetine for chronic musculoskeletal pain
Local anesthetics - Lidocaine patch (Lidoderm ®) and lidocaine / prilocaine cream (EMLA ®)
