File a Medical Provider's Report of Accident (ROA)
This should take 15 to 30 minutes.
Be done in 10 minutes! If the patient files the worker portion online or by phone, you only need to complete the medical portion of the ROA.
To protect patient information, you will be logged out after 20 minutes of inactivity. You cannot save your work until the ROA is completed.

- Choose your role:
- Injured worker or representative.
- Healthcare or other provider.
- Activate your account.
- Login to My Secure L&I.
- Choose from either service:
- File Worker Report of Accident.
- File Provider Report of Accident.
