New Customers: When you submit your Library Agreement Form, you can also send us a video reservation form with videos you want to reserve.

Start a video library account	Complete the Borrower Agreement Form (F417-205-000). Fax or mail your form to the video library.
Reserve a video	Call 360-902-5444 Or toll free (in Washington State): 1-800-574-9881 OR Complete the Video Reservation Form (F417-206-000) and fax or mail it to the video library.
