PGAP Activity Coaching

Becoming an Activity Coach Provider in Washington state

Who can provide these services?

The following provider types may apply to become approved activity coach providers for WA State L&I:

  1. Physical therapists.
  2. Occupational therapists.
  3. Vocational Rehabilitation Counselors (VRC)

Additional Requirements:

  1. Certificate of completion of the Progressive Goal Attainment Program® (PGAP) through Mick Sullivan
  2. Experience working professionally in worker’s compensation for 3 or more years and at least one year in Washington State
  3. Completion of suicide prevention screening and referral training that meets the WADepartment of Health standards per RCW 43.70.442.  Having an active WA OT/PT license meets this requirement.
  4. Access to a professional/private space which is ADA accessible.

How to apply to become an approved PGAP Activity Coach in Washington State

Send a packet to L&I that includes the following:

  1. Your full name, phone number and email address
  2. Company Name
  3. L&I Provider Number
  4. Physical address(s) of your office space
  5. Copy of your PGAP certificate of completion
  6. Describe your worker’s compensation experience
  7. For VRCs, also send a copy of your completed suicide prevention training or a letter stating your intent to meet this requirement within 6 months.
