Activity Prescription Form (APF)

L&I considers the timely and complete use of the Activity Prescription Form (APF) for patients with restrictions as an occupational health best practice. The form communicates a worker’s physical restrictions and ability to work as well as the provider’s treatment plans. In addition, workers' time-loss benefits are certified by the APF and your chart notes. Learn more and get started with APF data submissions.

Claims information

This transaction will allow providers to electronically send patient information with a claim number and get a response with the patient’s information, claim number, claim status, accepted diagnoses codes and accepted diagnoses descriptions for each claim submitted. This can be done in a batch of claims or individual claims. This will be a quicker and easier way to get information than through L&I’s current Interactive Voice Recognition (IVR) system.

Report of Accident (ROA)

Much like the APF transaction, the ROA will allow providers to send the initial notification of an illness or injury to L&I directly from their Electronic Medical Record (EMR) system using the HIE.
