Workers Severely Injured Using Demolition Robots (281.04 KB PDF)

Two construction workers were severely injured in separate incidents involving remote controlled-demolition machines also known as demolition robots. Workers operated similar machines with three-part articulating arms powered by electric-controlled hydraulics. Both used remote controllers intended to keep them outside the machine's risk zone, which varies by specific machine, attachment, and task.

The six crew members were working on the boat in different areas that were covered with plastic tarps in order to prevent paint chips and dust from entering the water.

Forklift Operator Dies When Crushed between Forklift Overhead Guard and Mast - Summary (381.24 KB PDF)

In May of 2012, a 61-year-old longshore worker operating a forklift died when he was crushed between a forklift's mast and the operator cab overhead guard. The incident occurred when he left the forklift operator's seat and climbed over the operator's console and stood on the dash between the mast and the overhead guard so that he could presumably look into a bin elevated on the forklift's forks. He did not turn off the forklift engine. As he was standing on the dash, his right foot moved backward and contacted the mast tilt control lever on the console. This caused the mast to tilt back, crushing his torso between the mast cross member and the overhead guard.

Tether Line Breaks after Moving Base Machine (569.44 KB PDF)

An operator with over 15 years of experience on a feller buncher and 4 months on a tether machine had a close call when one line of a dual line tether system broke after relocating the base machine. After finishing cutting a strip, the operator needed to relocate the base machine. There was a processor operator working near the base machine who was an experienced machine operator. The tether operator thought it would be easier to have the processor operator relocate the base machine.

Saw Cuts One Line on Dual-line Tether System (554.08 KB PDF)

In October 2017, an operator had a close call when he cut through one of the lines of his tethered machine. The operator of a dual line tether system was using a bunching head to fall large hemlock trees on 80% slope. He had 15 years of experience running equipment, and six years of experience working on the rigging. He had been operating this system for five months. The base machine had its bucket dug in and tracks parallel to the hill slope on a large landing (Figure 1). Chains connected to the tether machine were shackled to spliced eyes on the lines from the base machine. The lines were in lead with both machines and separated by stumps.

In March 2014, a falling maple limb struck and killed a 65-year-old timber cutter. The cutter and his employer, the owner of a logging company, were working at a logging site. The ground was flat and the day was rainy. The cutter was falling timber and the owner was operating a log loader to push trees over as the cutter was sawing them.

In September 2011, a 55-year-old log truck driver was fatally injured when he was struck and crushed by logs being loaded by a grapple loader onto the trailer of his truck. The victim worked for a trucking company that had been contracted to haul logs by the company logging the site where the incident occurred. The loader operator worked for the logging company. Both were experienced in their fields, but had not worked together before beginning work at this site approximately one week prior to the incident.

Two Propane Gas Supplier Workers Electrocuted when Boom Truck Crane's Boom Contacts 7,200 Volt Overhead Power Line (2.85MB PDF)

In May of 2010, a 40-year-old propane service technician and a 25-year-old material handler were electrocuted at a propane gas supplier's tank storage yard when a boom truck crane's boom contacted an overhead power line. The incident occurred as the service technician operating the crane was preparing to lift a tank. The two victims were employed by a large national retail and wholesale supplier of propane. They worked at their employer's propane service center and regional office location. This plant location provided a variety of propane services including bulk fuel delivery, tank and cylinder delivery and installation, service location fuel sales, and appliance sales, service, and repair.

On January 15, 2008 a 62-year-old equipment operator was fatally injured when the hydraulic excavator he was operating tipped over the side of a bridge, falling 60 feet into the river below. The operator was working on the demolition of the bridge by removing cut concrete slabs from the bridge deck. He was using the hydraulic excavator which was equipped with a type of bucket attachment (Slab Crab) designed for bridge deck and concrete slab removal work.

On September 27, 2009, a 38-year-old orchard worker was fatally injured when he was crushed between a motor grader and a semi-truck with a trailer. The semi-truck, which was at the orchard to pick up apple bins, was unable to maintain traction and got stuck while attempting to travel up a gravel orchard road with a 6-7% grade. The worker and the semi-truck driver were attempting to tow the stuck semi-truck with the assistance of a motor grader operator.

On January 29, 2010, a 27-year-old machinist was fatally injured when he was struck by a piece of round stainless steel bar stock that he was machining in a computer numerical control (CNC) lathe. According to the general manager, the victim was machining washers from the round bar stock for a gill net reel frame. He had placed a 6 foot piece of the bar stock into HAAS TL-3W model lathe. Approximately 3 feet of the steel bar stock extended past the spindle and out of the lathe and was unsupported. As the victim was machining the round bar stock, the 3 foot unsupported portion extending through the spindle bent to nearly an 80 degree angle.

In June of 2010, a 26-year-old female deck engineer died when she was struck by the counterweight of a barge mounted crane/derrick. The deck engineer (victim) and a crane operator, both employees of a marine construction company, were working from a crane/derrick barge in support of a crew working at a job site on a project to replace pilings underneath a pier. The project involved having divers in the water and workers in small boats taking out the old pilings and replacing them with new ones. The crane operator on the crane/derrick barge would operate the crane to lift waste pilings from the water and place them on an adjacent barge and lift new pilings and other supplies to workers in the water.

An assessment of gases in oxygen-deficient hay silos and the effects of forced ventilation.

Many modern tower silos used in the agricultural industry for hay and grain storage are oxygen-limiting by design. Forced-air ventilation using a forage blower is a method commonly used to decrease concentrations of toxic and asphyxiant gases and to increase the O2 content within a silo headspace prior to worker entry.
