To apply for a permit, you will need to submit a permit application and an elevator plan if necessary.

• Elevator Permit Application (F621-005-000): Allow 30 business days for a response.
  Note: This is also used for residential incline chair lifts and alteration permits.
• Construction Elevator Installation Application and Inspection Data Report (F621-001-000): Allow 2 weeks for a response.

Once you have filled out the form, you will need to calculate the fees for your permit application.

Note: There will be no permit refunds issued for permits that were not pulled with correct contract values or didn't have accurate alteration descriptions.
